Wait — is dry needling the same as acupuncture?

We get this question a lot. The short answer: they use the same thin needles, but that's where the similarity ends. Acupuncture is rooted in traditional Chinese medicine and works with energy pathways. Dry needling is based on Western anatomy and neuroscience — we're targeting specific muscle trigger points, nerves, and connective tissue to release tension and restore function.

When the needle hits a trigger point, you'll often feel a brief muscle twitch — that's the muscle releasing. It can feel intense for a second, but most patients describe the relief that follows as immediate and significant. And because we're addressing the actual source of the dysfunction — not just the symptoms — the results tend to last.

What we actually do — and why it works differently here.

At Pursuit, dry needling is always performed by your Doctor of Physical Therapy — not a tech, not an aide. That matters more than it might sound. Because the difference between dry needling that works and dry needling that doesn't usually comes down to understanding why that trigger point formed in the first place.

Is it coming from a movement pattern that's been loading one muscle too much? A postural issue from sitting at a desk all day? A compensation from an old injury your body never fully sorted out? We figure that out first. Then we needle. And then we address the root cause so the trigger point doesn't just keep coming back every few weeks.

We also use electrical stimulation (e-stim) combined with dry needling when appropriate — running a gentle electrical current through the needles to enhance the muscle release and speed up recovery. It sounds intense. Most patients find it deeply relaxing.

Dry needling works well for a lot of different things.

Here are the most common reasons people come to us specifically for dry needling:

Chronic muscle knots & trigger points — the kind that keep coming back no matter how much you stretch or foam roll. Dry needling releases them at the source.
Neck & upper back tension — especially the kind that builds from desk work and stress. If your traps are always tight and your neck always stiff, this is often the fastest relief you'll find.
Shoulder pain & rotator cuff issues — dry needling can significantly reduce the muscle guarding and tension that builds around a painful shoulder, making rehab exercises far more effective.
Low back pain — particularly when the pain is coming from overloaded paraspinal muscles or deep hip muscles like the piriformis. Often works where other treatments haven't.
Hip & glute tightness — for runners and active adults whose hips are always "on" and never fully recover. Dry needling gets into the deep tissue that's hard to stretch or foam roll effectively.

Headaches & jaw tension (TMJ) — trigger points in the neck, suboccipital muscles, and jaw are a surprisingly common source of chronic headaches. Dry needling these areas can provide relief that nothing else has.

Most people describe the needle insertion as a very brief sensation — often barely noticeable. When the needle hits a trigger point you may feel a quick muscle twitch, which can feel intense for a second. The relief that follows usually makes it very worthwhile. We always go at your pace and check in throughout the session.

Dry needling is based on Western anatomy — we're targeting specific muscle trigger points, nerves, and connective tissue based on your movement assessment and pain pattern. It's performed by your Doctor of Physical Therapy, not an acupuncturist.
It depends on what's driving the pain and how long it's been there. Some people feel dramatic improvement after one or two sessions. Chronic issues that have been building for months or years typically take more time. We'll give you a realistic timeline at your first visit — not a vague "let's see how it goes."
Some people feel a little sore for 24–48 hours after a session — similar to the feeling after a deep tissue massage or a hard workout. It's totally normal and usually resolves quickly. Drinking plenty of water and gentle movement afterward can help. Most people find the soreness is far outweighed by how much better the area feels once it clears.
We're a cash-pay practice, which means we don't bill insurance directly. We know many people have PT coverage through their employer — and we want to be upfront about that. What you get in return: a full hour, one-on-one with your DPT every session, no visit limits, and no pre-authorization delays.
